Glass breakage claims arise when damage of property (primarily automobile windows) is caused by the negligent acts or omission. In order to file a claim the damage must meet the $1000 deductible. All Departments are responsible for costs under the $1000 deductible. If damage is under the deductible contact the State contracted vendor, Capital Glass & Mirror at (225) 293-8440 to replace the glass.
To file a claim for glass breakage, please submit all the necessary information. To ensure your claim is processed in a timely manner, please submit all required information. The list below can be used as a guide:
Complete Glass Breakage Form
Police report if applicable
- Copy of driver’s license
- Copy of current registration
- Copy of current insurance
- Pictures of damage
Once your claim information is received, LSU Risk Management will submit it to the State Office of Risk Management for approval. We will contact you with the claim number and the name and number of the glass vendor once your claim is approved. You may choose to have the glass repaired/replaced with no out of pocket cost through the state’s glass vendor. You may also choose to have the glass repaired/replaced using your personal insurance or a glass vendor of your choice at your expense. Please provide proof of payment for any out of pocket expenses and we will submit to our insurance carrier for payment.
