Attention: In an effort to make the Institutional Review Board application process quicker and reduce the use of paper and filing space, we will soon stop accepting paper copies of IRB applications. Investigators will be required to submit their applications to the IRB office (email@example.com) or the Screening Committee member in their academic area by email. Continuation forms and modification requests should also be submitted by email. This requirement will begin on April 18, 2014. Until that date the IRB office will still accept applications both by paper copy or email.
The applications on the IRB website may be completed and saved electronically. If you still wish to complete the application by hand, you will need to submit a scanned copy. The application and accompanying documents (consent, instruments, etc.) should be attached to an email to the IRB office or the Screening Committee member.
Along with eliminating the use of a large amount of paper, it is our hope that this new process will help the IRB function more efficiently. Please feel free to contact the IRB office if you have any questions about this new process.
