Attention: As of April 18, 2014, the IRB no longer accepts paper copies of IRB applications, continuations, or modifications. All new applications should be submitted by email to the IRB office (firstname.lastname@example.org) or the Screening Committee member in your academic area. Continuation forms and modification requests should be submitted by email directly to the IRB office.
The applications on the IRB website may be completed and saved electronically. If you still wish to complete the application by hand, you will need to submit a scanned copy. The application and accompanying documents (consent, instruments, etc.) should be attached to an email to the IRB office or the Screening Committee member.
Along with eliminating the use of a large amount of paper, it is our hope that this new process will help the IRB function more efficiently. Please feel free to contact the IRB office if you have any questions about this new process.
